Dq1 Jenna Raila 1 posts Re: Topic 1 DQ 1 Accreditation by the Joint Commision is a commitment to excellence that helps to attract more patients and elevate the organization and the care that it provides. “Joint Commission standards focus on state-of-the-art performance improvement strategies that help health care organizations continuously improve the safety and quality of care, which can reduce the risk of error or low quality care.” (The Joint Commission, 2020). In 2019, the University of North Carolina at Chapel Hill was placed on probation by the Joint Commission for several problems including inadequate suicide screening documentation and assessment, lack of “ligature resistant hardware and furniture” in behavioral health areas and incomplete or inadequate documentation of physician medication orders. (Kommers, 2019). The UNC hospital system could have prevented the probationary period from occurring by setting standards for each of these issues and doing surveys to ensure these standards were being met and acting upon them when they were not. Another option for preventing probationary periods or loss of accreditation from the Joint Commission include creating mock Joint Commission committees that round different units randomly and ask similar questions that the Joint Commission would ask. This can help hospitals to ensure they are meeting the standards required to be accredited. References: Kommers, A.M. (2019). UNC Hospitals placed on probation by Joint Commission. Most of the time they are inspected and accredited by the Joint Commission. The Joint Commission which is a nonprofit group that evaluates hospital’s patient safety guidelines, quality of care and sets standards to be followed (Why Achieve Accreditation, 2020). When a patient sees that the hospital, they are at is accredited they can feel confident in the quality of care they are receiving. The Joint Commission is always looking for ways to improve patient safety. The Joint Commission can offer performance improvements that will reduce the risk of errors (Why Achieve Accreditation, 2020). In 2019 the Joint Commission placed the UNC hospitals on probation because of patient safety violations (Murphy, 2019). The Joint Commission was concerned that the physician’s orders were not filled out properly and the missing documentation of suicide screening of their patients (Murphy, 2019). This probation was caused by staff being careless in their treatment of their patients. This could be prevented by the staff taking a few extra minutes to make sure the physicians orders were filled out properly and to screen each patient about suicide. Management should randomly audit patient’s paperwork to see if the prescription was filled out properly and to see if the suicide screening was done. Why Achieve Accreditation. (2020). (2019). UNC Hospitals’ accreditation is at risk after problems found at facilities. https://www.newsobserver.com/news/local/article234315482.html DQ3 Crista Wilson 3 posts Re: Topic 1 DQ 1 Here in my home state of North Carolina, our Chapel Hill based UNC Hospital system was placed on probation in August 2019. Several local newspapers and the Becker’s Hospital Review indicate the probation period was for inadequate suicide screening documentation and assessments, and the lack of “ligature resistant hardware and furniture” in behavioral health areas, as well as incomplete or inadequate physician medication orders (Kommers, 2019). UNC Hospital system is accredited by the Joint Commission which holds the hospital accountable to high standards of patient care. Without this accreditation, these key improvements to care may not have been cited leaving room for serious risk or injury to the patient. According to the Joint Commission, one of the goals of accreditation includes improving risk management and risk reduction. By citing these items, the Joint Commission identifies areas for improvement and the facility is able to implement a corrective action plan allowing UNC hospital to keep their Medicare certification. Prevention is key to all things healthcare in my opinion. It appears the focus from this survey may have been the behavioral or mental health aspect of healthcare. In this case, all patient areas should be assessed closely for the safety of patients who have potential to harm themselves. This assessment and attention to detail may have caused the facility to overcome the citation for improper hardware and furniture in the behavioral health areas. Suicide screening documentation and assessments were also named inadequate in this survey. Suicide rates in my area are on the rise and particular attention must be paid to those coming to the hospital admittedly needing help. Yearly competencies to assess the skills of the documentation and assessment staff along with continuing competence through a guest speaker or online training could help prevent these types of citations. The final citation was inadequate physician medication orders. A large hospital system like UNC should have a developed risk management program. Grand Canyon University HLT 308 Guidelines to Protect Patients & Employees Responses Physician orders that are inadequate should be reported through the risk management program’s monitoring system and followed up on closely for additional education or re-education of software programs or simple order entry tasks. A review of all orders placed by physicians should be reviewed for quality for a set amount of time to ensure the issue has been resolved (and continues to stay that way until the next review by the Joint Commission). Kommers, Anne-Marie, 2019. “UNC Hospitals Placed on Probation by Joint Commission: The Joint Commission Has Placed Chapel Hill, N.C.-Based UNC Hospitals’ Accreditation Status on Probation after Finding Several Problems during a Recent Survey, According to The News & Observer.” Becker’s Hospital Review, Aug. 2019, www.beckershospitalreview.com/quality/unc-hospitals-placed-on-probation-by-joint-commission.html. DQ4 Christina Hannah 1 posts Re: Topic 1 DQ 1 The Joint Commission is the accrediting body for healthcare facilities. They are committed to patient safety and quality care. Being nationally accredited by the Joint Commission, makes a facility stand out as a top medical center because of their state-of-the-art performance improvement strategies to improve patient quality care and safety (Why Achieve Accreditation, 2020). It makes the hospital more marketable to potential patients and doctors. In May 2020, Virginia Mason Medical Center in Seattle, Washington, was denied full accreditation after an unscheduled visit from Joint Commission found 29 safety violations (Joint Commission, 2020). The visit came after 650 patients may have been exposed to Hepatitis B (Joint Commission, 2020). Joint Commission found 29 violations, including, medications not being labeled, fire drills not being conducted properly, and medical equipment not being inspected or maintained accordingly (Joint Commission, 2020). Preventing these infractions could be done by implementing a few processes to catch any violations before Joint Commission conducts an inspection. Grand Canyon University HLT 308 Guidelines to Protect Patients & Employees Responses Discussion: Guidelines to Protect Patients & Employees Responses. One such thing could be conducting fire drills more often and without notice. Another way to aid in prevention of violations, and I worked at a facility that did this, is to conduct mock surveys monthly. This will aid in catching any potential violations early and being able to correct them before inspection by the Joint Commission. -Christina Hannah References: Joint Commission denies Virginia Mason full accreditation after discovering safety issues, 2020.
