Note: Remember that you are an ambassador for Capella University. Your professional appearance, behavior, and preparation should reflect positively on the organization. • Explain your proposed data review project and the course requirements to the client. • Redefine your performance indicators and outcome measures, as needed. • Negotiate potential changes to the project that will enhance its value to an organization, while meeting the requirements of the capstone course. • Obtain client approval for your data review project. Scheduling the Meeting After identifying your first choice for a point of contact in the organization, you must contact that person and schedule a meeting. The meeting can take place in person, via web conference, or over the phone. Be prepared to contact a second person if you are unable to meet your first choice. Note: Face-to-face interaction is essential for developing a strong professional network. An inperson meeting is strongly encouraged. However, if you are unable to meet with the client in person, consider hosting an online meeting to increase the level of engagement in the call. Initiating Client Contact You are encouraged to use the following sample talking points for initiating contact with the health care organization: My name is [insert your name]. I am a learner in the master’s program on health care administration at Capella University. I am currently enrolled in the capstone course, in which learners help a health care organization examine a real-world issue or problem. Assignment: Client Meeting and Project Approval The topic must be one in which I can examine the existing information or data, either from a practicing health care professional or perhaps the data that has already been collected. Potential examples include patient satisfaction surveys, compliance with a new regulation, patient safety initiatives, or evaluating process deviations for a new service line offering or system, such as admitting home care patients onto the service. I am offering to volunteer my services to draft a project proposal, complete a data review, and provide recommendations on how to address the issue. These recommendations will include insights from current peer-reviewed health care journals. 1 Would you be willing to work with me on a project that examines data on [insert topic here; for example, compliance with the new hospice reimbursement system]? This would involve a meeting with me for 15–20 minutes to provide feedback on how I would examine and organize the data. Then I would review the data, review the relevant, authoritative literature, and provide a project report, with my findings, at the end of the course. Would this be helpful, and if so, may I contact you via e-mail to send a copy of the draft proposal after my instructor has approved it? Thanks so much for your time, [Your name] Client Background and Communication Preferences Consider the following questions before you meet with the client: • Have you thoroughly researched the organization’s website and the professional background of the client? Have you obtained any other information necessary to provide perspective on the organization’s potential needs? • Is the client’s organization currently undergoing any major change that might have an impact on your proposed project? • Does your client prefer to meet with you over the phone, via web conference or chat room, in person, or via another modality? • What time or setting is ideal for the client? Presenting Your Project Proposal At the meeting, present the project proposal you developed in Assessment 2, and explain your role as a master’s degree learner offering a data review project as a part of your degree completion requirement at Capella University. • Explain your proposed project and the course requirements. Explain that you are seeking the client’s approval to review the existing data. Answer any questions. • Describe briefly what type of data you will be examining, what performance indicators you will be examining, and how you are proposing to measure each indicator.Assignment: Client Meeting and Project Approval In addition, explain how you will visually display your summarized data (for example, as a pie chart, graph, spreadsheet, and process map). • Communicate to your client any statistical or other software that may be needed for the data analysis. • Explain how you will measure the outcomes for each of the four areas of the balanced scorecard. Refining Your Project Proposal After presenting your proposal, request feedback from the client on the scope and feasibility of your project. In addition, attempt to identify examples of evidence-based support for selected key performance indicators, determine whether you can gain access to additional related data sets, and verify the accessibility of data sets that could be made available for you to review. 2 The meeting is an opportunity to work with your client to redefine the performance indicators and outcome measures that you propose to examine. It is also an opportunity to make changes that will be of mutual benefit and enhance the outcome of the project. Before approving your data review project, the client may have a few suggestions to enhance your data analysis, such as adding a performance indicator, using a specific benchmark, or using an existing assessment tool. The client may also want to add something to your project for inclusion in the final report or may want you to spend more time on the project than is allotted. In such instances, you must negotiate a solution that meets both the course requirements and your client’s needs. It is your job to verify, discover, or redefine your performance indicators and outcome measures so you can proceed to your data review. Consequently, you may wish to initiate a preliminary discussion with your client on the following points: • Units of service. • Types of units applicable to your project. • Selection of units. • Output performance measures: o Scope definition. o Service completion. o Client outcomes. • Quality dimensions. • Four types of outcome measures and rationale for your selected measures. • Process for selecting outcome measures, such as stakeholder input and assessment. • Use of standardized measures. • Use of performance measurement information. During the meeting, you may discover the following: • You have chosen the wrong indicators for your problem statement. • The indicators are not included in the organization’s data sets. Assignment: Client Meeting and Project Approval • The client thinks that different indicators might be more helpful. • There is limited data on a performance indicator. • There are incomplete entries or lack of consistent coding of data. • There are mismatched data sets from which you have to generate a meaningful summary. Obtaining Project Approval Agree to submit a revised project proposal, which integrates the client’s feedback and any negotiated changes before you end the meeting. Try to obtain a commitment from the client to sign and return the approved proposal so that you may proceed with the data review. 3 Submit the revised proposal to the client for final approval. Ensure that your client signs the revised copy of your proposal. Meeting Follow-Up Although it is not required, you may choose to follow up with your client via e-mail or phone after completing the assessment. This would be an ideal time to extend your thanks for reviewing your initial proposal. You may offer to e-mail the client a draft of the project report that you will complete in the next assessment. Upon receiving the report, the client has the option to review the type and purpose of the data you are analyzing, how you will organize it, and how the data will be presented in the final report. Even though the course does not require you to receive feedback from the client about your completed report, the person you interview is the professional audience your data review project is intended to serve. This person is very likely going to be interested in seeing your completed report and could serve as an informal editor of your deliverable for this course. 4 3/16/2020 Client Meeting and Project Approval Scoring Guide Client Meeting and Project Approval Scoring Guide CRITERIA NONPERFORMANCE BASIC PROFICIENT DISTINGUISHED Identify the appropriate health care professional in an organization from whom to seek approval for a proposed project. The problem of focus in this project, therefore, is the high readmission rates as well as medication error rates in Mount Sinai when compared to the national averages. This problem affects all patients but more so harms minorities such as children and the elderly. The problem of hospital readmissions, therefore, presents a challenge to delivering quality care in the hospital. The purpose of this project is data examination. The process of data examination is informed by the need to understand the nature of readmission rates and medication errors at the hospital for the purpose of future improvements. The method to be used will be a data review of existing data in the hospital to determine the current rates of readmission as well as medication errors. The outcomes will be used in improving the quality of care by reducing the risk of readmission in the hospital and increasing accuracy in medication. This proposal presents an overview of the background of the problem as well as the nature of this problem. It also presents an analysis of the value of this project and what the healthcare organization stands to gain from it. Outcome measures are analysed to highlight the specific means of measuring the projected indicators and display the data during examination. Lastly, this proposal presents a timeline and highlights how the project is connected to leadership for the learner. DATA REVIEW 4 Introduction I am examining the rates of unplanned hospital readmission because high unplanned hospital readmission rates indicate gaps in health care which might be the cause of various problems that influence patient outcomes and cost of healthcare (Wadhera et al., 2018). I would like to examine the patient outcomes and will assess this data by measuring unplanned readmission rates, medication errors, and hospital-acquired infections as outlined by the Centers for Disease Control and Prevention (CDC) and Centers for Medicare and Medicaid Services (CMS). This project will add value to Mount Sinai Hospital, New York, by highlighting areas for patient care improvement, methods of improving the patient experience, and the potential for reducing costs and litigation risks. The project aligns with my career and professional goals by providing a chance to engage with practical health improvement project for the benefit of patients. Statement of the Problem Mount Sinai St. Luke’s hospital has 30-day unplanned hospital readmission rates of 15.40% between July 2017 and June 2018 as compared to the national average of 15.30% as defined by New York State Department of Health (NYS Health Profiles, 2018). Background Scholars have researched the effects of patient outcomes such as hospital readmission on healthcare. One of the most significant studies is in the correlation between medication errors and readmission rates. Pellegrin et al. (2017) studied readmission rates in Hawaiian hospitals and found that 26% of readmissions were related to medication errors. This study highlights the impact of pharmacists in the healthcare system and presents an analysis of how patient outcomes are influenced by readmission and medication errors.Assignment: Client Meeting and Project Approval The study relates to DATA REVIEW 5 the topic of hospital readmissions by showing that many of the readmissions can be prevented through prevention of medication errors (Pellegrin et al., 2017). It thus connects medication errors to readmissions as major patient outcome issues. Similarly, in a study outlining the impact of person-related, environment-related, and communication-related factors on medication errors, Manias et al. (2019) highlight that admitted patents and family are able to alert health care staff on errors only 15% of the time. This figure shows that medication errors are highly likely to harm the patient since a majority of patients may not identify them. Manias et al. (2019) present one main measure of outcomes which is of interest in this project and hence highlight the risk of failing to reduce medication errors on patient outcomes. Moreover, the importance of understanding and measuring readmission rates in a hospital has also been highlighted by Roberts et al. (2018). In the study, the authors found that hospitals serving higher-risk patients may have a higher rate of readmission which is related to the population rather than the quality of care. These outcomes highlight intersection between health care quality and patient population. Hospitals serving high-risk patients may have high readmission rates hence the need for the hospital to take measures to conquer the effects of the exacerbated risks (Roberts et al., 2018). In another study, Bailey et al. (2019) studied the general characteristics of hospital readmission. The study concluded that blood diseases have the highest rate of readmission. Additionally, when reviewing trends on readmission, it was determined that the rates for minors (age 1-20) were on the rise while the elderly maintained the overall highest readmission rates (Bailey et al., 2019). With hospital readmission rates for minors on the increase, there is a need for improved healthcare and efforts to reverse this trend which seems to be mostly affecting minority populations. These studies highlight readmission rates as a crucial component of healthcare. DATA REVIEW 6 Lastly, the burden of HAIs has been studied widely and the need for interventions clearly highlighted. In a study initiated by the CDC, it was determined that 1.7 million patients acquire HAIs annually in the U.S (Haque, Sartelli, McKimm, & Bakar, 2018). These figures are worrying because one in 17 of these die due to the HAI. In another study, the impact of HAIs in the length of stay in hospitals has been studied. The outcomes show that HAI significantly increases the length of stay with an average of 10.4 days (Zaha et al., 2019). This literature shows that the three outcome measures for patient outcomes are crucial in healthcare and should be studied. The studies by Haque et al. (2018) and Zaha et al. (2019) highlight the role of HAIs n influencing patient outcomes and raise further questions on the connection between HAIs and readmission rates. Assignment: Client Meeting and Project Approval This question has been tackled by Shah et al. (2014) who identify surgical site infections (SSI) as the leading cause of hospital readmissions. In this article, the authors identify the connection between HAIs and readmissions and hence find a positive correlation between the two. This literature has highlighted the problem of medication errors as well as hospital-acquired infections as they affect readmissions in hospitals. The literature highlights the seriousness of hospital readmission as a measure of patient outcomes hence necessitating further analysis. Value Proposition Mount Sinai Hospital, NY, is one of the oldest hospitals in the country and its services have benefited millions of Americans. The mission of the hospital is to “provide compassionate patient care with seamless coordination and to advance medicine through unrivalled education, research, and outreach in the many diverse communities we serve” (Mount Sinai, n.d.). This project presents the tools for assessment on how the care of the organization can be said to be compassionate and its effects on patients. By reviewing patient outcomes, the hospital can improve care and the patient experience.
