[SOLVED] Progressive Care
Clearly detail your one on one patient health care and how it relates to clinical reasoning skills. I have dealt with the patient when I was working in a progressive care/ COVID unit at Harborview medical center. ever since I have worked there, I have developed many skills such as listening to the patients and also be more supportive through their tough time here at the hospital. There was very unexpected that happened while I was on duty at work. I was taking care of a patient who was very difficult because she was very agitated, she was not a primary English speaker. she was not able to talk to anyone, she was refusing everything, even the medication. Even the nurse I was working with did not know how to react to this situation. I remember going into her room trying to do the vitals but was refusing in the beginning, I tried to talk to her in the primary language but was not talking at first. So I started communicating with her, asked her about her family in her language, it made her feel like she was in a safe environment, I asked her what she likes to eat and how does she want to eat? She was very comfortable with me because creating a comfortable environment for the patients is really important. She took all her medication with me there and was a lot happier later which made me realize that I am really good at communication because I was very shy and could not hold the conversation. but I realize that I had the potential all along but was never able to unleash it. I have worked with COVID positive patients. they get agitated cuz they want to get out of the room that they are stuck in. I have seen patients who were giving up on their lives and were in comfort care. but meanwhile talking to them and engaging in their conversation, I have given that support in order for them to fight for their lives and I have seen them getting better and discharging with no COVID. I have death with schizophrenic and dementied patients and learned that the very important thing with those patients is how you approach them, you have to be ready for every phase when it comes to working with schizophrenic patients.

[SOLVED] Diabetes Self Management
Develop a professional product to improve care or the patient experience related to the identified health problem (DIABETES SELF MANAGEMENT) with a 2-4 page summary of intervention findings, evidence, and the best-practice basis for the professional product. A brief summary of the findings of your intervention and evidence-based support for your professional product should accompany your product. Demonstration of Proficiency By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria: Competency 1: Lead people and processes to improve patient, systems, and population outcomes. Explain ways in which leadership of people and processes was utilized while designing an intervention and implementation plan. Competency 2: Make clinical and operational decisions based upon the best available evidence. Justify decisions related to developing a professional product with relevant research, evidence, and best practices. Competency 3: Transform processes to improve quality, enhance patient safety, and reduce the cost of care. Demonstrate process improvements in the quality, safety, or cost of care as a result of direct clinical intervention and a developed professional product. Competency 7: Implement patient-centered care to improve the quality of care and patient experience. Demonstrate patient-experience improvements resulting from a clinical intervention and implementation of a newly developed professional product. Competency 8: Integrate professional standards and values into practice. Communicate professionally in writing that is clear and logically organized, with correct grammar, spelling, and use of APA style. Instructions For this assessment, you will develop and deliver a professional product to address the health problem defined in your first assessment to improve care and the patient experience. This will be delivered remotely rather than face-to-face to the individual or group that you have identified. Appropriate examples include the development of a community education program focused on a particular health issue or a handout to help the elderly and their families understand their Medicare and Medicaid options. The product must be useful in a practice setting, relevant to your project, and designed to improve some aspect of care or the patient experience that can be used in your own practice, with your family or community. Relevant products include but are not limited to Patient education handouts (such as a medication sheet). Patient safety plan. Process improvement in-service. Medicaid/Medicare patient coverage and finance guide. Teaching plan. Your submission will consist of two parts: an APA-formatted paper providing your rationale and supporting evidence, and the professional product. For the brief APA-style paper, one organizational option is: Summary of intervention and implementation: What did you do? How did you lead in terms of the processes used in the project? With what participants? Who were your interprofessional collaborators? How did you lead them in this project? What were your key findings? The evidence-based rationale for professional product development: What professional product did you develop? Why did you choose this type of product? How does it align with your intervention? How will the product help improve outcomes? How will the product enhance the patient experience? How does the product improve a process related to quality, safety, and/or cost of care? How do relevant research, evidence, and best practices support your choice of professional product and approach? For your professional product, you may choose to develop a deliverable that could be used in your care setting to communicate and sustain relevant improvements related to the intervention you carried out. The key is that the professional product is useful to the target audience (such as a large-print Medicaid/Medicare patient coverage and finance guide that is easy for the elderly and their families to use, or an easily implemented process improvement in-service for home care nurses). Scoring Guide Criteria Your brief paper and professional product will be assessed according to the following scoring guide criteria: Explain ways in which leadership of people and processes was utilized while designing an intervention and implementation plan. Demonstrate process improvements in the quality, safety, or cost of care as a result of direct clinical intervention and a developed professional product. Demonstrate patient-experience improvements resulting from a clinical intervention and implementation of a newly developed professional product. Justify decisions related to developing a professional product with relevant research, evidence, and best practices. Communicate professionally in writing that is clear and logically organized, with correct grammar, spelling, and use of APA style. Additional Requirements Length of submission: APA-style paper should be 24 pages of content plus title and reference pages. There is no length requirement for your professional product. The type of deliverable you choose will determine its length and format. A number of references: Cite a minimum of 5 sources of scholarly or professional evidence that supports your decisions and rationale in your APA-style paper. Resources should be no more than 5 years old. APA formatting: References and citations are formatted according to the current APA style. Use appropriate APA citations (in-text and reference list) in the paper and professional product
[SOLVED] Nursing Home and Subacute Care
Nursing Home and Subacute Care During the nineteenth and the twentieth century, the term “nursing home” was synonymous with long-term care. Although today the exclusive use of the term “long-term care” is no longer accurate for nursing homes, it continues to remain and will not change easily. However, newer terms such as “nursing facilities” will help clarify the role of specific long-term care organizations and will differentiate them from others in the health care industry. Subacute care is a relatively new but rapidly growing medical service in the continuum of care. Today, it is considered the fastest-growing segment of the health care delivery system. Research the online references such as EBSCOhost, SocINDEX, Cumulative Index to Nursing and Allied Health Literature (CINAHL), or PubMed for information on nursing facilities and subacute units and respond to the following questions: Do you agree with the change in the terminology of nursing homes to nursing facilities? Why or why not? How do the terms relate to each other? What impact does the historical perspective of the nursing home have on the stigma related to the quality of care? How were nursing facilities developed? What have been the consequences of the change in terminology? Do you think the change in terminology will impact the quality of care in the future? If yes, how? If no, why? What changes do you see nursing homes making in the future in order to keep up with the ever-changing needs of the demographics of seniors? What are subacute units? How did subacute care emerge? What are the strengths and limitations of the emergence of subacute care in long-term care as related to issues in levels of patients’ acuity (various levels of nursing care based on the needs of patients)? Support your answer with relevant examples. What is the impact of subacute care on the cost and quality of care? Do you think subacute care needs to be an integral component of hospitals, or should it be an integral part of the long-term care system? Provide a rationale for your answer.
