Some of the additional information, in this case, may be ethical questions that may help the medical practitioners to make a better decision. For instance, it is good to check the directive in her file chart to see if it contains relevant information. It is also good to clarify her mental health history from the family. Her medical history would be more insightful and it would make her treatment easier. The son should say if he has more information about her apart from the notarized directive. It is also good to check the hospital database to find out whether there is more information to help in this case. Given that her son came with a directive it is possible that she was taking an overdose with alcohol so that she could commit suicide. All the same, there is no evidence for this. Contacting her primary care provider can be effective as he or she would provide more information about her.
How would each of the three approaches to ethical theory respond to this case?
The ethical approaches that can be used in this case are consequentialist, virtue ethics and deontological. Deontologist looks at the wrongness or rightness of making a choice. Consequentialism focuses on the consequences of a person’s actions. The virtue ethics looks at the person’s characters.
As earlier stated, deontological is that which puts its focus on rightness and wrongness. In this scenario, this is applicable to the matter of treating JoEllen given that her condition has worsened. One can be in a dilemma in this case since she has given directions not to be placed on a life machine yet that is what is required to save her life. As a healthcare provider, one cannot be sure of the right thing to do. However, the best thing to do is to save her life. On the other hand, it is good to think of her statement. One may assume that she did that probably due to an influence of alcohol. It is possible that the health directive was signed when she was of sound mind.
Consequentialism focuses on the consequences of a person’s actions which helps a person to make a decision. As such, this would be applicable to what followed after she took an overdose. By the time she was saying that she is not supposed to be here, she was not of sound mind (Gallagher, & Haworth, 2015). Taking the drug while drunk could have altered her decision. One cannot be sure by the statement “Not supposed to be here…” it could mean a hospital or else to be alive.
Virtue ethics shows the character of someone. One may look at her character by the choices she had made. It is possible that she took the overdose for suicidal purpose or because of the influence of alcohol. The virtue ethics here may apply to the healthcare practitioner. There is a need to make the right decision by applying medical ethics. Morally, the nurse can be able to provide the life support so to try and see if they can save her life. However, given that there was a notarized advance directive, this may not be legally acceptable especially if anything happens to her. The nurse may assume it is an attempted suicide and tries to save her life but the directives prevent her to do so.
What are the advantages and disadvantages of each approach?
Deontological has some advantages and disadvantages as well. One advantage is that it allows one to have moral breathing room. Another advantage is that one will have the intuitive knowledge of wrong or right. It helps people to do away with some immoral actions. It allows people to be objective as they can act in any way so long as it is not prohibited (Holyoak, & Powell, 2016). However, some of its disadvantages are that it may conflict with moral duties. There is also a reduction of morality to some extent of avoiding bad actions.
One of the advantages of consequentialism is that there is equal opportunity for people to share their happiness thus making decisions more objective. It also provides a rule where people are able to base the ethical decisions (Quong, 2018). One disadvantage is that the physicians are forced to predict the consequences. Another disadvantage is that it excludes the minority. It is also possible that people get subjected to moral laws they are not in agreement with.
One advantage of virtue ethics is that it helps people to feel motivated such that they work on morality. Individuals are also able to map their moral reasoning. The main focus is building oneself into better individuals rather than focusing on theory. Some of the disadvantages are that they change with time thus lacking a definitive definition (Van Hooft, 2014). It does not give clear direction on what to be done on moral dilemmas. It as well has no concerns about right or wrong actions.
What is the right thing to do? Justify your position with clear reasoning.
The right thing to do here is to follow the directives as given not to place her on the life support. This can be supported by the theories we have discussed. we have seen that our morals shows us what is right and wrong and therefore to make a decision one must consider the laws, consequences and health directives. JoEllen only stated a few words and no one clearly understands what she meant. In this case, the best thing to do is to follow the notarized directive that was brought by her son. It is possible that her son was bringing this since they knew that she was not sober at the time and this could help the doctors to make the right decision.

There are little accurate statistics on the frequency of mental emergencies in general and family practice, in general hospital emergency rooms, or among patients handled by emergency medical teams.
The prevalence rate of mental emergencies has been found to range from 10% to 60% in various research (2).
This large range of results could be due to a number of methodological flaws.
Given the current realities in medical care, as well as the public’s overall antipathy to mental problems of any kind, it’s not surprising that psychiatric emergencies are rarely treated in specialized psychiatric facilities.
Mentally ill people who don’t want to be stigmatized frequently go to general hospitals’ emergency departments, which are usually easy to find and open 24 hours a day.
Prevalence
In non-psychiatric institutions such as general hospitals and general medical practices, the prevalence rate of mental emergencies has been reported to range from 10% to 60%.
According to a retrospective study conducted at Hannover Medical School (Medizinische Hochschule Hannover, MHH), mental patients presented to the emergency room at a rate of 12.9 percent in 2002. (3).
Psychiatric emergencies accounted for 12 percent to 25% of all emergency cases seen by emergency medical services (4, 5).
In 10% of cases, general practitioners and family physicians, the most widely regarded providers of primary care, saw psychiatric emergencies.
Regardless, there are few credible data on this topic from German-speaking countries, and disparities in health-care systems between countries may restrict the generalizability of findings from one country to the next (6, 7).
As a result of the foregoing, all physicians require a basic understanding of the diagnostic and therapeutic procedures to be followed in mental emergencies.
A number of studies have indicated that up to 60% of mental problems presenting to medical attention in mostly non-psychiatric facilities and hospitals are not effectively diagnosed or treated (2, 8).
Acute excitement with psychomotor agitation and self-destructive or suicidal behavior are the two basic forms of psychiatric emergency.
The purpose of this article is to present the diagnostic and differential diagnostic features of these entities, as well as treatment options.
The algorithms in Figures 1 and 22 are meant to aid in diagnostic and therapeutic decision-making, but they are not required to be followed to the letter in every situation.
Because there is a scarcity of high-quality research in emergency psychiatry, the algorithms and treatment suggestions presented here should be taken as expert guidance rather than absolute truth.
They are based on the authors’ clinical experience and relate to contemporary psychiatric emergency management at the University of Bochum’s Psychiatry Department (Germany).
Every physician, on the other hand, should be conversant with the fundamentals of psychiatric emergency management, which are discussed in the next section.

What Are the Nursing Profession’s Origins?
Nursing as a profession began in the mid-nineteenth century.
Florence Nightingale, a well-educated British woman, is credited as the creator of modern nursing by historians.
By becoming a nurse, Nightingale defied social expectations — even her affluent parents.
The thought of women breastfeeding strangers was frowned upon at the time.
However, Nightingale saw nursing as a unique potential for women.
She felt that by combining their education and scientific expertise, they would be able to improve patient care while also achieving personal autonomy.
The British government solicited Nightingale’s assistance at a military hospital in Turkey during the Crimean War in 1854.
The fatality rate of British soldiers dropped drastically within weeks of her small team’s arrival.
The public was impressed by Nightingale’s achievements, and she eventually persuaded the Western world of the dignity and worth of educated nurses.
Florence Nightingale, who defied social standards and argued the necessity of educated nurses, is credited with founding modern nursing.
A decade later, during the American Civil War, the evolution of the nursing profession quickened.
Women, often mistresses or wives of soldiers, began following the troops when war erupted in the North and South.
They were primarily responsible for the care of sick troops.
The most common infections were pneumonia, typhoid, diarrhea/dysentery, and malaria.
Mary Ann Bickerdyke was a renowned nursing volunteer.
Despite the fact that she was not married to a soldier, she accompanied Union Gen. William T. Sherman’s army.
Bickerdyke took care of 2,000 men on her own after the 1863 Battle of Lookout Mountain in Tennessee.

Saleema had total hip replacement in March 2015. One week later, the stitches started to bleed. She was concerned and went to the doctor’s office and was told it was a superficial hematoma. The bleeding continued on for three weeks and involved two doctor’s appointments and one trip to the emergency room. She was sent home each time with a diagnosis of a superficial hematoma.
Finally, Saleema’s visiting nurse saw her on a home care visit and called an ambulance, concerned about the amount of blood loss. Upon further investigation at the ER, a terrible infection was discovered that had spread into the bone. Saleema went in for emergency surgery and a PICC line was placed in order to administer the medication vancomycin. On this medication, she developed Steven’s Johnson’s syndrome, which was not diagnosed right away as the skin peeled off her hands. She ultimately went into acute renal failure and had to go back to the hospital. Two days post-admission, she developed C-diff and several vancomycin-resistant infections. She died in the hospital, two months after the original surgery. The cause of death was determined to be septicemia. Saleema lost her life over what was supposed to be a routine hip replacement.
