The client disclosed something to you, they did not “agreed to spill the beans.” Similarly, you also need to be specific In your clinical descriptions for instance, you stated that his father “passed on due to depression.” How did he die? Did he die by suicide? People do not die from “depression.” They may have died because of suicide related to depression, but depression in and of itself is not a cause of death. Also, you stated that he cannot maintain relationships-why? Does he have a disordered personality? Does he become a violent with his intimate partners?”
