Diagnosis of depression requires careful consideration since 50% of the clinicians fail to recognize the signs and symptoms among patients. The failure of signs and symptoms identification leaves a lot of patients untreated. Depression may occur at any age, but it is mostly in adults as compared to children. Some people may fail to go for medical help, and those who seek medical attention receive inadequate care due to a lack of clinical knowledge. Lack of ongoing depression screening provides minimal information in determining and evaluating treatment efficacy. In 2016, the U.S Preventive Service task Force gave priority to depression in adults by recommending all adults to be screened. The move intended to improve health quality among patients (Siu & the US Preventive Services Task Force, 2016). Improved screening help realize the problem early and increase outcome due to better treatment. Through the use of the PHQ-9 tool, major symptoms are identified. The Problem The major problem identified in depression screening in Outpatient Mental health Clinic using a PHQ-9 screening tool is that the rate of diagnosis and treatments are still low as expected. So, Henderson’s need theory provides a framework that helps the health professional in the advance clinical practice increase diagnosis and treatment of the patients. Failure to identify the cause of depression is a missed out opportunity that reduces improvement of care quality (Beard et al., 2016). Henderson’s need theory is important in helping patients increase independence and ability to perform the activities that promote health without clinical help. The need theory has the goal of helping define the unique focus of the nursing practice. The four-component that build up the theory include an individual, health, environment, and nursing. So, in improving quality care, using the PHQ-9 tool guided by the need theory defines the practice that improves timely diagnosis and treatment. Individual The first component in the need’s theory is the patient, who is seen as a component of basic needs. Human needs require assistance to achieve health independence at one point. So, Henderson suggested that the body and mind cannot be separated, and both contribute to an individual’s overall health (Nursing Theory, 2020). So, the patient considers biological, sociological, spiritual, and psychological aspects for health needs. The theory suggests that patients should be treated as a sum of biophysical needs. Understanding the components that make up human needs can help advance clinical nurses considering depression screening as part of clinical assessment. In solving the problem in the outpatient mental health clinic, making the depression screening as part of the clinical assessment can increase the diagnosis cases and reduce the number of patients who do not get connected to the right treatment. The clinical nurse focuses on the four health needs before allowing the patient to go through the clinical assessment. Among the health needs include psychological health needs that need to be met. When it became part of the organization to test psychological problems for every patient in the outpatient facility as required by the U.S Preventive Service task Force, there will be improvement of care. The need theory’s psychological needs suggest that 10 and 14 needs are psychological and the health professional needs to ensure they are considered for the patient to be self-reliant and independent. The 10th need suggests that nurses need to help the patient independence in communicating with others in giving opinions, needs, and expressing their fear (Nursing Theory). When a person is depressed, they show signs of social isolation, anger, and negative emotions. The 14th need help in learning, discovering, or curiosity that enhances normal development and health (Nursing Theory, 2020). So, under the improvement of care through screening depression to the outpatient, it results to timely connect the people who need help with the right source that can improve psychological needs. The theory aid in advance care practice patient assessment. The nurses used a dynamic and systematic way of collecting data from the person, which is the first step in nursing care. In depression screening, the assessment includes the psychological factors and helps in the formulation of diagnoses. Ahtisham and Sommer (2015) conducted a case study of a patient who had depression. They had previously failed to comply with the provided procedures and guidelines for improving her well-being. As a result, the patient’s condition deteriorated, and she could no longer continue caring for herself independently or participate in health promotion activities. Henderson’s need theory was applied to the patient. It was a great success in helping the person improve her socialization with the family and community and follow the clinical guidelines of improving health. Using the PHQ-9 tool helps determine the person who is under is in depression. Depending on the score a person gets after answering the questions, the need theory takes over in helping the nurses determine the patient’s behavior (Licorish, 2018). Further, the theory guides nurses in increasing the administration of the PHQ-9 to patients who visit the facility. Increased administration will help reduce the unattended cases and improve the ability to connect people with the right help. Also, during the assessment stage, apart from administering the PHQ-9, the nurses can engage the patient further to get more details about the level of depression identified (Beard et al., 2016). Adequate information is important in determining that mental help can help understand the situation and the environment that the patient is in and how it contributes to the situation. So, if the environment contributes to the depression, the patient is guided on learning new living patterns in the same environment. As suggested by the theory, in achieving the level of independence, the patients must live freely in the environment with the ability to independently. Improving awareness of depression is the central aim of screening depression using the PHQ-9 tool. Quality care improvement on mental health starts by creating awareness on symptoms and signs related to mental illness (Beard et al., 2016). Sometimes it may be hard to realize a patient is suffering unless they go through the test. That is why there is a lot of missed opportunities in the outpatient mental facilities, and many people are left without any treatment. The needs presented in the theory improve awareness among the nurses, health professionals, and among the patients on what health needs should be met. When all the participants are aware of the importance of mental health wellness and its effect on general health, it increases compliance and administration of the PHQ-9. Health The theory defines an individual’s health independent functionality with aid from the health profession or family (Nursing Theory, 2020). So, the advanced clinical setting must stress activities that promote the clients’ health at all times. For the cases that require treatment, the right cure needs to be provided. The health component aims to get the patient into normal development. It may be challenging to promote health since several factors contribute to affecting mental health (Licorish, 2018). The theory offers the solution to the problem of low administration of depression screening by proving factors to consider in assessing, analyzing, and treating the person. In some cases, the level of depression may hinder the person from taking responsibility for health promotion activities. According to the tool, a person who rates severe depression needs expert help and is required to follow the guidelines provided to them. The nurses may be limited in action if the person fails to show up for the follow-up sessions. In such a scenario, the theory can apply the spiritual and sociological factors in improving the patient’s health. Nursing The theory defines nursing as an act of improving care with the knowledge, will, and strength they have. The nurses take patient’s need and making sure they get better, as their responsibility in (Nursing Theory, 2020). Advance Clinical nurses aim to improve care and engage practices that improve performance and reduce mental illness cases among the outpatient. There are many nurses in the critical pathway who use their strength in effectively improving patients’ outcomes. The consistent use of the standardized screening tool improves the rate of identification, diagnosis, and treatment. As a result, the patients will be improved and improve the holistic care. Since the nurses spent a lot of time with the patient, they play a great role in recovery and promotion among patients. There needs clinical advancement, particularly among nurses as they ensure the delivery of holistic care. The holistic care nursing approach requires a mutual understanding of physical, psychological, spiritual, and emotional (Gligor & Domnariu, 2020). Advancement of care emphasizes the nurse and the patient’s partnership, which improves the healthcare negotiation leading to recovery. Similarly, depression screening of outpatient mental health clinics requires a mutual understanding with the clinical and patient to increase trust. More importantly, the improvement of holistic care is addressed in Henderson’s need theory by providing different aspects that the clinical nurse considers in improving health and ensuring lead recovery. Therefore, the theory advocates for the right steps that can be used in the mental health clinic to ensure that nurses focus on administering the PHQ-9 test before allowing people to get out of the facility. If the test is not administered, the clinical nurse should consider it a failure to offer holistic care. Conclusion In conclusion, the problem faced by the advocacy of depression screening among adults is diagnosing and treating more cases. Many people are not screened for depression and leave many people untreated or delayed to receive medical attention. As healthcare, the focus on holistic care, increasing diagnosis of mental issues is paramount and the goal of holistic care. Through advanced clinical practice, the PHQ-9 is in helping self-assessment among patients and is simple to use. The health professional can know the patients whose mental health is at risk and those that need monitoring. So, it reduces the financial, health, and economic burden related to mental health. More importantly, the use of Henderson’s need theory provides a framework through its four components in ensuring a high level of diagnosis and treatment with the help of the diagnosis tool.
