The UNC hospital system could have prevented the probationary period from occurring by setting standards for each of these issues and doing surveys to ensure these standards were being met and acting upon them when they were not. Another option for preventing probationary periods or loss of accreditation from the Joint Commission include creating mock Joint Commission committees that round different units randomly and ask similar questions that the Joint Commission would ask. Most of the time they are inspected and accredited by the Joint Commission. The Joint Commission which is a nonprofit group that evaluates hospital’s patient safety guidelines, quality of care and sets standards to be followed (Why Achieve Accreditation, 2020). When a patient sees that the hospital, they are at is accredited they can feel confident in the quality of care they are receiving. The Joint Commission is always looking for ways to improve patient safety. The Joint Commission can offer performance improvements that will reduce the risk of errors (Why Achieve Accreditation, 2020). In 2019 the Joint Commission placed the UNC hospitals on probation because of patient safety violations (Murphy, 2019). The Joint Commission was concerned that the physician’s orders were not filled out properly and the missing documentation of suicide screening of their patients (Murphy, 2019). This probation was caused by staff being careless in their treatment of their patients. This could be prevented by the staff taking a few extra minutes to make sure the physicians orders were filled out properly and to screen each patient about suicide. Several local newspapers and the Becker’s Hospital Review indicate the probation period was for inadequate suicide screening documentation and assessments, and the lack of “ligature resistant hardware and furniture” in behavioral health areas, as well as incomplete or inadequate physician medication orders (Kommers, 2019). UNC Hospital system is accredited by the Joint Commission which holds the hospital accountable to high standards of patient care. Without this accreditation, these key improvements to care may not have been cited leaving room for serious risk or injury to the patient. According to the Joint Commission, one of the goals of accreditation includes improving risk management and risk reduction. By citing these items, the Joint Commission identifies areas for improvement and the facility is able to implement a corrective action plan allowing UNC hospital to keep their Medicare certification. Prevention is key to all things healthcare in my opinion. It appears the focus from this survey may have been the behavioral or mental health aspect of healthcare. In this case, all patient areas should be assessed closely for the safety of patients who have potential to harm themselves. This assessment and attention to detail may have caused the facility to overcome the citation for improper hardware and furniture in the behavioral health areas. Suicide screening documentation and assessments were also named inadequate in this survey. Suicide rates in my area are on the rise and particular attention must be paid to those coming to the hospital admittedly needing help. Yearly competencies to assess the skills of the documentation and assessment staff along with continuing competence through a guest speaker or online training could help prevent these types of citations. The final citation was inadequate physician medication orders. A large hospital system like UNC should have a developed risk management program. The visit came after 650 patients may have been exposed to Hepatitis B (Joint Commission, 2020). Joint Commission found 29 violations, including, medications not being labeled, fire drills not being conducted properly, and medical equipment not being inspected or maintained accordingly (Joint Commission, 2020). Preventing these infractions could be done by implementing a few processes to catch any violations before Joint Commission conducts an inspection.
