He has also reviewed Medicare’s Hospital Readmission Reduction Program (HRRP). He now has a better appreciation about the challenges of care coordination for individuals experiencing one or more chronic conditions. He realizes, based upon the data and discussions with health care system leaders, that the current models of medication management, primary care, and specialist office visits aren’t working for his clients with complex health care problems. To learn more about why the current models of care delivery aren’t working, Simon decides to get more information from a couple people who deal with complex health care problems every day. He schedules times to meet with the Coleen, the VicePresident of Population Health, and Albert, an experienced home care nurse. Simon Meets with Coleen, Vice-President of Population Health Simon: Hi, Coleen. Thank you for meeting with me. I’m trying to figure out why so many of my complex care management clients are being readmitted to the hospital within 10 to 20 days. I’ve talked with several folks and found out that there are a variety of reasons. One of the big ones is that the current models of care delivery aren’t working. I thought you could help me understand why, and what could be done to help. Coleen: Right on! Happy to talk about this stuff. Health care needs nursing leaders like you to use data and experience to identify problems. You’re doing the right thing by talking with industry experts and using evidence-based practices to gain a better understanding about the problem and their causes. That’s what we do on a larger scale in population health. We monitor data, such as readmissions, and focus on the population of individuals who represent the majority of those readmitted. Our current data shows, just as yours has, that folks with chronic conditions such as diabetes, congestive heart failure, and chronic lung disease, represent the highest percentage of readmissions. Coleen: What we’ve discovered is that these populations don’t have a primary care provider, they’re on more than 10 medications per day, they have no or limited understanding about how to self-manage their conditions, they don’t have anyone taking responsibility for care coordination, and English usually isn’t their first language. Simon: Very interesting. These seem like insurmountable problems. How can they be addressed? Coleen: You’re right. These problems can be insurmountable, if we continue to try to address them in traditional ways. Health care needs innovative solutions to address these problems. These solutions include ways to use technology to connect these populations to providers. We’ve successfully used digital technologies (mobile or web-based) focused on disease selfmanagement, telehealth, virtual visits, e-visits, patient portals, continuous glucose monitoring, and patient wearable sensors to address access to care and self-management. Simon: I love technology. I’m so happy to know that it’s a solution to these problems. Coleen: I love technology, too, Simon, but it’s not the sole solution. Technology needs knowledgeable, caring, understanding, and creative folks to make it work. I want you to meet a person who demonstrates all these attributes. Albert is an experienced BSN-prepared nurse who has worked in home health care for many years. He’s found innovative ways to address such problems as readmissions by not only using technology, but also by coordinating care for his patients and families and connecting them to community resources. Let me call him for you now. He may be in the field. Are you okay meeting him on the road? Simon: Of course. I can’t wait to talk with him. Simon Meets with Albert, Homecare Nurse Albert: Hi, Simon. Great meeting you! Coleen told me you wanted to learn more about solutions to health care problems like readmissions. Simon: Thank you so much for meeting with me, Albert. I realize you’re very busy. Coleen had high praise for all your innovative efforts to use high touch and high tech to address problems like readmissions. I can’t wait to hear more. Albert: I’m honored to be asked, Simon. I really don’t see what I do as being innovative, I see it as being a patient-centered, culturally competent, caring nurse who not only remains up-to-date on evidence-based practice, but also uses critical thinking and a systems approach to problem solving. Simon: That sounds great, Albert, but how do you do that? Albert: Great question. Let me give you an example. Several of my patients—my population if you will—have chronic lung conditions, like COPD, and experience shortness of breath on a regular basis. They’re frequent ED visitors, and are often readmitted for COPD exacerbation. They stay in the hospital for a couple of days, are treated with steroids, nebulizers, and antibiotics. They’re discharged to home on oral antibiotics, tapering doses of steroids, inhalers, an appointment to follow up with a pulmonologist, and a home care consult. They go home with reams of paper, with instructions about the medications and how to manage their shortness of breath. What I find when I make my first visit is that the medications from the most recent hospital visit sit on a table with the medications from previous hospital stays, and the printed instructions are piled on top of the reams of paper from past hospitalizations. My patients usually haven’t read the information and are confused about which antibiotic to continue, which inhalant to use, and how to taper the steroids. They know nothing about what triggers their shortness of breath, how to measure their oxygen saturations, or how to get the appropriate assistance to help them live with their COPD.
