INTRODUCTION This report will analyze and compare the hospital’s data in Hawaii to the national average and provide recommendations on how to improve the hospital outcomes. All of the data in this report is taken and calculated by using Microsoft Excel and Microsoft Access. As a quality improvement (QI) analyst, my main purpose of conducting this report is to look at and understand how discharges and average length of stay (ALOS) in Hawaii in 2008 may affect their quality of care comparing to the national average indexes. Moreover, from the top 5 DRGs ranked by discharge and ALOS, we can learn more about other aspects of care as well as some diseases that are more likely to occur due to demographic and geographic issues. As a QI analyst, I can use that information to make decisions on how to implement certain methods to keep the cost as low as possible while providing the highest quality of care. According to the Census Bureau, in 2008, Hawaii’s population was approximately 1,229,000 people in which 85% was under age of 65 (Kaiser Family Foundation, 2019). According to Greg Moody and Sharon Silow-Carroll, health management associates, from the Common Wealth Fund, in 2009, among other states in the U.S., Hawaii was considered as having the healthiest population (Moody and Silow-Carroll, 2009). In terms of healthcare accessibility, Hawaii had “higher rates of insurance coverage than other states, mainly because of a 1974 employer mandate to provide insurance for employees” (Moody and Silow-Carroll, 2009, p. 4). Therefore, it is critical to conduct this report in order to understand the difference in different aspects relating to healthcare outcomes between Hawaii and the national average. LOS is an important measure for management, financing, and resource utilization (Freitas et al, 2012). As in 2008, Hawaii’s ALOS was around 6 days which was 1 day higher than the national average of 5.36 days. The difference may due to the fact that Hawaiian older population (60 years old and above) tend to grow faster than the younger one (Substance Abuse and Mental Health Services Administration, 2012). Therefore, the LOS for the elderly tends to be longer than younger adults. Moreover, for all working Hawaiians who are under 65, they are covered under the Prepaid Health Care Act of 1974, where it requires “all employers to provide COMPARISON OF HEALTHCARE OUTCOMES 5 health insurance to their employees who work 20 hours or more a week for four consecutive weeks” (Moody and Silow-Carroll, 2009, p. 4). Thus, patients who are covered under this act tend to spend more days in the hospital than their counterparts in other states. As a QI analyst, it is essential to note that LOS is an important measure to the hospital and organization’s success in achieving the primary goal of providing the highest quality of care while minimizing costs and other related factors. “Reducing LOS can release capacity in the system (including beds and staff time) and improve throughput, enabling the hospital to serve more patients” (HealthCatalyst, 2016, para. 6). The difference may due to the fact that almost 90% of adult and nearly 95% of children was under health insurance coverage (Moody and Silow-Carroll, 2009). Moreover, health care cost and health insurance premiums in Hawaii were much lower than any other states and even the national average. For instance, “Hawaii is […] the lowest employer-sponsored health insurance premiums of any state ($3,831 for individuals in 2008), about 13 percent below the national average [and] Medicare costs per beneficiary are 31 percent below the national average” (Moody and Silow-Carroll, 2009, p. 4). Another demographics and ethnical/racial aspects that also contribute to the lower cost per discharge in Hawaii is that Hawaiians are considered as having very good health and having good accessibility to the gatekeepers as well as preventive care (Moody and Silow-Carroll, 2009).
