She was diagnosed with depression after the death of her husband three years ago, and ever since then; she has been battling with her symptoms of depression. The loss leads to her difficulty in concentrating, fatigue, sleeplessness, and withdrawn from the activities that she previously enjoyed prior the death of her husband. She also reported that the thoughts of her husband death intensified the symptoms of depression. She was on Nardil 15mg by mouth three times daily, but she was non-compliant due to some WEEK 8 JOURNAL ENTRY 5 intolerable side effects severe sleeplessness and involuntary muscle movements, so her psychiatrist discontinued the medicine. Mental status: She is aware of her condition. Denies hearing voices or seeing things no other person can understand. Level of participation: she participated actively in the therapy and was able to share about the death of her husband. Mood: She reported feeling depressed and isolated from friends sometimes. Suicide/ violence risk: She also reported having the thoughts of harming herself but no plans Change in coping/ ability skills: Her younger sister is the strong support system she has who accompanies her to sessions and keeps her company. Speech: Normal tone and respond to questions appropriately. Pertinent medical history: History of asthma Psychiatric history: Depression Medication: Nardil 15mg by mouth three times daily, Differential diagnosis (DDX): Her diagnosis of depression aligns with the DSM-5 based on the symptoms the patient reported which includes sustained feelings of sadness, loss of interests in activities she once liked, fatigue, difficulty in concentrating, trouble in sleeping, and loss of appetite (American Psychiatric Association, 2013). Efficacy of the Existential-Humanistic Method The existential-humanistic approach holds that patients can make their choices as well as self-awareness. When applying this approach to deal with the patient, it will enable the patient to find her philosophical meaning while facing her depression condition. Based on the method, the WEEK 8 JOURNAL ENTRY 6 fundamental issues the patient experiences are drawn from her anxiety and sadness resulting from the passing of her husband and some of those issues that deprive her of making sound judgment that creates meaning in her life. NURS6640 Cedar Valley College Wk 8 Depression Psychotherapy Diagnoses Therefore, the aim of engaging this patient in this therapeutic approach is channeled toward enhancing her self-actualization while building her natural ability for self-direction (Shaikh & Kauppi, 2015). It demonstrates that the strategy will enable the therapist to influence the patient to make informed decisions that will empower her to manage her depression condition. Legal and Ethical Implications When dealing with patients, especially, those with mental health complications, therapists should maintain the aspect of confidentiality, and it can be achieved by preventing unauthorized individuals from gaining access to the information of the patient. Some situations may force the therapist to breach confidentiality, and this can happen if the patient shares information that demonstrates malice against those around him or her or even shares suicidal thoughts to her therapist (Cuijpers, Karyotaki, Pot, Park, & Reynolds, 2014). Sharing the limits of confidentiality with the patients would be integral and will reassure the patient that her information cannot be shared freely except if there is a court order to release her information. Part 2: Reflection on Clinical Supervision Experiences Dealing with patients with various mental health disorders has expanded my knowledge in the area of counseling clients compared to my initial encounter at my practicum site. My preceptor supervised me while practicing my professionalism in dealing with the clients with various mental health complications. At the end of this practicum experience, my preceptor commended my progress in the utilization of therapeutic approaches we discussed to empower the patient’s sense of well-being. She also recommended different methods of counseling WEEK 8 JOURNAL ENTRY 7 difficult and non-compliant clients. I will implement those strategies during my next quarter as I will still be working with her for my winter semester practicum. The clinical platform allows me to practice and implement what I learned in school into the work environment. Also, the therapeutic sessions broadened my skills in psychotherapy, and while dealing with patients during the sessions, I noted that those sessions that seemed almost impossible, was eventually successful in empowering those patients to function very well and be able to manage their mental health issues (Williams et al., 2018). My preceptor also informed me of the necessity of the therapeutic approaches when integrating them to deal with the patients. In summary, I will translate what I have gained in the sessions in the real work environment by augmenting my assessment skills based on the mental issues affecting the patients and establish the appropriate treatment plan that will improve their quality of life. Conclusion Treating patients with mental disorders is always challenging but engaging the proper therapeutic approaches enables the therapists to deal with them in a way that eventually improves their cognitive functioning. While the therapeutic strategies have been tailored towards addressing mental health concerns, they ideally employ distinct methods. A therapeutic approach can be proved to be useful if it has been noted to deliver exceptional results which in this case, entails addressing the mental health disorders in patients. The therapists can employ different approach to influence their patients to make informed decisions that seek to contain their mental health concerns. It is also integral for the therapists to adhere the ethical and legal considerations because they usually present them with a guide on how to deal with the patient.
