We need to always consider the patient’s cultural and spiritual considerations when educating them and their family (Johnson, 2018). We need to make sure they have access to not only physical support but psychological support as well. We need to be educating patients and informing them of the web-based resources so they have information at hand. Oxygen, wheelchairs, ramps, canes, and medication education are a must for these patients (Johnson, 2018). Helping them to maintain an independent lifestyle to the best of their ability while keeping our patients as safe as possible. Making sure our patients follow and adhere to medication routine as well as using all information given to provide them with a thorough education will decrease readmission and help them to live as independently as possible.
