[SOLVED] Poor Communication

 From your experiences, describe 1) an instance when poor communication made a situation bad or worse and 2) an instance when good communication made a situation better. Without revealing personal information, be as descriptive as possible, telling what was going on and how the situation was made bad/worse and better, respectively.
