During this video I plan to: Reflect on an interdisciplinary collaboration experience noting ways in which it was successful and unsuccessful in achieving desired outcomes. Identify how poor collaboration can result in inefficient management of human and financial resources supported by evidence from the literature. Identify best-practice leadership strategies from the literature, which would improve an interdisciplinary team’s ability to achieve its goals Identify best-practice interdisciplinary collaboration strategies to help a team to achieve its goals and work more effectively together. I’ll make mention of authors from the literature. Being a nurse I have had experiences being a part of an interdisciplinary team that collaborated to brainstorm, test, analyze and implement a new policy within my unit. Working on a Well-Baby postpartum unit on night shift is very rewarding but can also have its challenges. When I had first started it was the postpartum nurse’s responsibility to bring a wheelchair, IV pump and belonging cart from the fourth floor down to the third floor to pick up the newly delivered mother baby couplet. I inquired after my orientation with my manager why the nurse who has a patient assignment of two or three couplets, meaning 4-6 patients is the leaving the unit for 30 minutes upwards of an hour to bring up the new couplet. She responded with that was always how it was done but she can see how that might be an issue. At that point the ball started rolling on changing the standard of the postpartum nurse physically transferring the new couplet to having the Labor nurse transfer the couplet to the postpartum floor. Having to complete a change like this would require engaging interdisciplinary staff members(Director of Women’s and Children’s, Unit Managers, Charge Nurses and Staff Nurses.) Utilizing all the staff allowed us to work in interprofessional collaboration.The published evidence would support my idea. Quoting from a 2015 publication from American Journal of Nursing, “The World Health Organization (WHO) released a blueprint for implementing interprofessional education and collaborative practice to meet demand in the face of a global health care workforce shortage. In the WHO framework, collaborative practice happens when multiple health workers from different professional backgrounds work together with patients, families, careers and communities to deliver the highest quality of care.”To sum that up, interprofessional collaboration is an organized example of teamwork in my opinion. Quoting from a 2017 publication from Nursing Management, “The team delivers intentionally coordinated care based on the latest evidence, individualizes care based on patient needs, and leverages each other’s documentation to prevent duplication and errors during care transitions.”Strategies for Managing Human and Financial Resources Essay. The goal of our concept was to make transferring care of patients easier, more time efficient while also preventing errors in the process.I will now go step by step through the Plan-Do-Study-Act process. I’ll refer to that as the PDSA.Quoting from a 2017 publication from Nursing Education Perspectives, “The Plan, Do, Study, Act (PDSA) cycle for continuous quality improvement was used for concept evaluation.” Being able to continuously assess this plan/concept was useful to make sure this is a good idea between all units, the PDSA was the best way to test our plan. I will begin with Plan We initially had a meeting with the Nursing Unit Managers first to make sure everyone agreed that this was something they would like to see changed in the units. The initial push back was if labor is too busy to bring people up, the postpartum nurse needs to come down and get them. The managers had agreed that in circumstances of the entire unit being too busy to bring a patient up that the postpartum nurse will come downstairs and get the patient. Once all concerns for both units were addressed and worked out we then scheduled a meeting with the Nursing Director of Women’s and Children’s for our hospital. She had thought it was a great idea and that it would only benefit the patients and improves patient safety and quality. The difficult part was getting the labor charge nurses and staff nurses on board with the proposed idea. They understandably were not happy about the new idea considering their workload was being increased. When the management teams explained the reasoning along with patient safety and that needing to be the priority the labor nurses agreed that them bringing the patients up to the fourth floor after they delivered made the most sense. Now I will examine the ‘Do’ phase of the PDSA Two weeks after we had the meeting it was time to implement the new idea. All of the staff was aware that it was going to start this Sunday for any delivery needing to be brought up after 0700. When there was only 15 minutes left of each recovery the labor and delivery nurse would call the postpartum nurse and notify her that they were planning on bringing up the patient and making sure it was ok to bring them up. Calling ahead allowed each nurse to plan accordingly for when the patient was being transported and being transferred from one care team to the other. Now I will look at the Study Phase After the first full week of this implemented idea the unit managers and charge nurses met to review how the week had gone with the new responsibilities now in affect. The only issues that had occurred was labor forgetting to call postpartum with enough time to be prepared for the patient, which resulted in labor having to wait until the nurse taking report came to the room instead of being readily available and waiting for their arrival. The managers had identified why this worked well the first week and hopefully would continue to be a successful change. The labor nurses need to call the postpartum nurse at least 15 minutes before initiating the transfer of patients. The postpartum nurse must verify that the labor nurse had documented the infant weight so the infant is able to be transferred to postpartum. Having the postpartum nurse in the room when the new patients are brought up helps the new patient feel welcome but also takes into consideration being conscious of the labor nurse’s time while off of her unit. If the admitting postpartum nurse was unavailable to be there the charge nurse would start the admitting process until the nurse was able to get to the room. Upon reflection of this experience I would say utilizing the PDSA model was successful in getting our new policy launched. It was great being able to work with a different unit and putting the patient’s safety first. I think since this experience with both units was so positive it will only help future ideas being accepted by both units easier.
