This week I started with the concept labs, which I found helpful. Then I moved to the reading and Im currently working on the assessments. I found Jessica Nishikawas YouTube assessment video to again be helpful as an example of how the assessment should flow. With shadow health I have a hard time getting the answers I am looking for with my questions, as well as knowing how much to educate and empathize. In person it is much easier to read the patient and educate and empathize when you can see they need it or there is an opportune moment to educate. Im trying to get better at reminding myself to use those buttons and switch from the question to those more often. Ive used the made incredibly easy series of books previously in nursing school and they also are proving to be helpful now as well. As noted in the forward of the book, every topic from examining the retina to percussing liver borders to auscultating for a pericardial friction rub is described step by precise step. I have watched Jessica Nishikawas YouTube assessment video for the HEENT assessment we completed but have not watched any other ones yet. The ones I have found very helpful for me are the RegisteredNurseRN on Youtube. I completed the respiratory one and am watching the Cardiac one at this time, she actually has two for this assessment. Im continuing to use thePoint for the skill assessments along with both of our text but the area I keep struggling with is the questions. I think I have asked everything and then I have not gone in-depth enough. I do appear to be getting better at educating and empathizing with Tina, but this is not something Im as concerned with as I do interact differently when I am with a real person. One of the challenging aspects for me has been the headings given to us. In all the provider documentation I have read the format is different. In my mind at least, I have been practicing an advanced physical for quite some time. I have that down fairly well in my head. Then when I look at our sections I feel lost. I have found numerous flash cards and pocket guides for physical exams. I have done numerous literature searches and was floored when I read one article that mentioned the newer clinicians feel that the physical exam is outdated (Kelly, Freeman & Dornan, 2019). Something else mentioned in this article is that newer clinicians are not practicing physical exams, which makes them less proficient, thereby leading to higher rates of misdiagnosis and medical costs.
