American Sentinel N522PE Module 6 Assignment 4 Sample Solution for Discussion 6 This weeks readings cannot be more dissimilar. While I cannot say that I am an expert on either topic, I do sit on my facilitys mental health environment of care checklist (MHEOCC) committee. This committees primary goal is to assess the inpatient mental health units environment to identify ligature and suicide prevention safety issues (U.S. Department of Veteran Affairs, 2018). Essentially the team puts themselves in the suicidal Veterans mindset to identify hazards that could cause them and staff harm. This process has reduced the suicide rate by 2.64 per 100,000 to 0.87 per 100,000 admissions (U.S. Department of Veteran Affairs, 2018). I encourage you to utilize this checklist in your facilities. In addition to these concepts, I have learned about the use of the Columbia-suicide severity risk scale (C-SSRS) (Substance Abuse and Mental Health Services Administration [SAMHSA], 2014). Suicide is the second leading cause of death for those 10 to 25 years old (Erbacher & Singer, 2018). The use of a validated tool such as the C-SSRS provides a standard practice to assess a patients intent to commit suicide (Erbacher & Singer, 2018). Warning signs for risk include ideation, purpose, and a plan (Erbacher & Singer, 2018). At my facility, we use this tool to trigger further intervention and implement a patient focus safety plan. In medical practice, screening questions include depression, anxiety, substance abuse and personality disorders (Bickley & Szilagyi, 2017a). While knowledge of assessment processes for suicide and the mental health environment is essential; I would not rank myself as adept at mental health patient-centered education. A female patients health history includes menarche, menstruation, and menopause (Bickley & Szilagyi, 2017b). Information about pregnancy, vulvovaginal symptoms, sexual health, pelvic pain, and sexually transmitted diseases continue to be essential for assessing female well-ing (Bickley & Szilagyi, 2017b). Necessary to determine patients ages 20 to 24 years for sexually transmitted infections such as chlamydia (Bickley & Szilagyi, 2017b). Monitoring for depression in women of reproductive age key after childbirth and neonatal loss (Centers for Disease Control and Prevention [CDC], 2020). Clinical factors, including a family history of inherited medical conditions and gene disorders, are critical components of assessing female reproductive health (American College of Obstetricians and Gynecologists [ACOG], 2020). I enjoyed reading your post and the MHEOCC sounds so interesting! At my facility we utilize the Ask Suicide-Screening Questions (ASQ) for ages 11 years old and up. We screen these patients either in the ER or on the inpatient unit if a direct admit from home. The ASQ is a simple 4 question tool that is able to identify those at risk for hurting themselves. If the patient answers yes to any of the 4 questions, they are immediately accompanied by an unlicensed individual to ensure their safety. In addition to their 1:1 status, a psych consult is placed, all belongings are removed from the room, the patient changes over into paper gowns, and the plastic trash bags are replaced by paper bags. Horowitz et. al. (2012) reviewed the statistical significance in utilizing the ASQ screening tool that the Joint Commission approved the use of this tool for all ages. Eight hundred twenty thousand new gonorrhea infections occur annually in the United States (Centers for Disease Control and Prevention, 2015). Gonorrhea case rates in South Dakota last year were 220.5 per 100,000 population, a 65% increase from the year prior (South Dakota Department of Health [SDDOH], 2019). Women commonly have asymptomatic carriage (Dains et al., 2016). Purulent discharge presents at the transitional and endocervical columnar cells (Dains et al., 2016). The patient presents with painful urination due to inflammation of the Bartholin glands and Skene ducts (Dains et al., 2016). Untreated women can develop the pelvic inflammatory disease with abscess formation and chronic pain in the pelvis (Centers for Disease Control and Prevention [CDC], 2019). Symptoms in men include dysuria (CDC, 2019). Men present with a white, green, or yellow discharge from the urethra (CDC, 2019). Some will have scrotal or testicular pain and epididymitis (CDC, 2019). The symptoms show up to 14 days after initial infection (CDC, 2019). Men and women can have rectal infections that include itching and discharge (CDC, 2019). Many people have an asymptomatic infection (CDC, 2019). Patients with pharyngeal infection present with a sore throat (CDC, 2019). References Centers for Disease Control and Prevention.
