High unplanned hospital readmission rates may indicate gaps in health care which might be the cause of various problems that influence patient outcomes and cost of healthcare (Wadhera et al., 2018). I would like to examine the patient outcomes and will assess this data by measuring unplanned readmission rates, medication errors, and hospital-acquired infections as outlined by the Centers for Disease Control and Prevention (CDC) and Centers for Medicare and Medicaid Services (CMS). This project will add value to Mount Sinai Hospital, New York, by highlighting areas for patient care improvement, methods of improving the patient experience, and the potential for reducing costs and litigation risks. The project aligns with my career and professional goals by providing a chance to engage with practical health improvement project for the benefit of patients. One of the most significant studies is in the correlation between medication errors and readmission rates. Pellegrin et al. (2017) studied readmission rates in Hawaiian hospitals and found that 26% of readmissions were related to medication errors. This study highlights the impact of pharmacists in the healthcare system and presents an analysis of how patient outcomes are influenced by readmission and medication errors. Similarly, in a study outlining the impact of person-related, environment-related, and communication-related factors on medication errors, Manias et al. (2019) highlight that admitted patents and family are able to alert health care staff on errors only 15% of the time. This figure shows that medication errors are highly likely to harm the patient since a majority of patients may not identify them. Moreover, the importance of understanding and measuring readmission rates in a hospital has also been highlighted by (Roberts et al., 2018). In the study, the authors found that hospitals serving higher-risk patients may have a higher rate of readmission which is related to the population rather than the quality of care. These outcomes highlight intersection between health care quality and patient population. In another study, Bailey et al. (2019) studied the general characteristics of hospital readmission. The study concluded that blood diseases have the highest rate of readmission. These studies highlight readmission rates as a crucial component of healthcare. Lastly, the burden of HAIs has been studied widely and the need for interventions clearly highlighted. In a study initiated by the CDC, it was determined that 1.7 million patients acquire HAIs annually in the U.S (Haque, Sartelli, McKimm, & Bakar, 2018). These figures are worrying because one in 17 of these die due to the HAI. In another study, the impact of HAIs in the length of stay in hospitals has been studied. The outcomes show that HAI significantly increases the length of stay with an average of 10.4 days (Zaha et al., 2019). This literature shows that the three outcome measures for patient outcomes are crucial in healthcare and should be studied. Value of Proposed Project Mount Sinai Hospital, NY, is one of the oldest hospitals in the country and its services have benefited millions of Americans. The mission of the hospital is to “provide compassionate patient care with seamless coordination and to advance medicine through unrivaled education, research, and outreach in the many diverse communities we serve” (Mount Sinai, n.d.). This project presents the tools for assessment on how the care of the organization can be said to be compassionate and its effects on patients. By reviewing patient outcomes, the hospital can improve care and the patient experience. As a healthcare leader, my mission is to improve the experiences of patients within the healthcare system. High unplanned hospital readmission rates may indicate gaps in health care which might be the cause of various problems that influence patient outcomes and cost of healthcare (Wadhera et al., 2018). I would like to examine the patient outcomes and will assess this data by measuring unplanned readmission rates, medication errors, and hospital-acquired infections as outlined by the Centers for Disease Control and Prevention (CDC) and Centers for Medicare and Medicaid Services (CMS). This project will add value to Mount Sinai Hospital, New York, by highlighting areas for patient care improvement, methods of improving the patient experience, and the potential for reducing costs and litigation risks. The project aligns with my career and professional goals by providing a chance to engage with practical health improvement project for the benefit of patients. Key Performance Indicators and Outcome Measures Patient outcomes include a range of broad performance indicators and outcome measures.Assignment: Data Review Project ProposalIn this proposed database review, however, patient outcomes are limited to a few performance indicators and their respective outcome measures. The key performance indicators in this project will include indicators of the structure measures and the outcomes of the treatment. These three categories of key performance indicators highlight the entire process of determining patient outcomes and consequently the quality of patient care in the healthcare setting. The outcome of treatment is a key indicator of outcomes because it predicts how effective treatment has been. The outcome measure to be used with this performance indicator is the 30-day unplanned hospital readmission rates. This data will be obtained from NYS Health profiles and CMS. Wadhera et al. (2018) explain that unplanned readmission is a predictor of patient satisfaction as well as cost of care. Therefore, based on the inconvenience of going back to the hospital as well as spending more on the same, this will be a great approach to understanding patient outcomes. The outcome measures to be used for this indicator are the number of medication errors per 1000 patients as well as rates of hospital-acquired infections (HAIs). One of the most significant studies is in the correlation between medication errors and readmission rates. Pellegrin et al. (2017) studied readmission rates in Hawaiian hospitals and found that 26% of readmissions were related to medication errors. This study highlights the impact of pharmacists in the healthcare system and presents an analysis of how patient outcomes are influenced by readmission and medication errors. Similarly, in a study outlining the impact of person-related, environment-related, and communication-related factors on medication errors, Manias et al. (2019) highlight that admitted patents and family are able to alert health care staff on errors only 15% of the time. This figure shows that medication errors are highly likely to harm the patient since a majority of patients may not identify them. Moreover, the importance of understanding and measuring readmission rates in a hospital has also been highlighted by (Roberts et al., 2018). In the study, the authors found that hospitals serving higher-risk patients may have a higher rate of readmission which is related to the population rather than the quality of care. These outcomes highlight intersection between health care quality and patient population. In another study, Bailey et al. (2019) studied the general characteristics of hospital readmission. The study concluded that blood diseases have the highest rate of readmission. These studies highlight readmission rates as a crucial component of healthcare. Lastly, the burden of HAIs has been studied widely and the need for interventions clearly highlighted. In a study initiated by the CDC, it was determined that 1.7 million patients acquire HAIs annually in the U.S (Haque, Sartelli, McKimm, & Bakar, 2018). These figures are worrying because one in 17 of these die due to the HAI. In another study, the impact of HAIs in the length of stay in hospitals has been studied. The outcomes show that HAI significantly increases the length of stay with an average of 10.4 days (Zaha et al., 2019). This literature shows that the three outcome measures for patient outcomes are crucial in healthcare and should be studied. Value of Proposed Project Mount Sinai Hospital, NY, is one of the oldest hospitals in the country and its services have benefited millions of Americans. The mission of the hospital is to “provide compassionate patient care with seamless coordination and to advance medicine through unrivaled education, research, and outreach in the many diverse communities we serve” (Mount Sinai, n.d.). This project presents the tools for assessment on how the care of the organization can be said to be compassionate and its effects on patients. By reviewing patient outcomes, the hospital can improve care and the patient experience. As a healthcare leader, my mission is to improve the experiences of patients within the healthcare system.
