While I am an A student in almost all the nursing theory classes, I feel that applying the knowledge during work is my weakness, and I lose self-confidence. I think that the gap here is the practice. The more I practice, the faster I gain the confidence back. Therefore, to gain professional confidence, my goal is to analyze one patient’s chart each clinical day for nine weeks and start making connections between their disease(s) process, lab values, history, and medications. To achieve my goal, I will select a patient with a disease or a condition that I have a little knowledge about; then, I will go over as many details as possible in their chart and start analyzing them and connect the possible dots to make sense. By doing so, I will gain enough knowledge to build my nursing professional confidence when I face similar future cases. Another weakness I need to improve is communication. SBAR communication strategy is a very important tool to communicate with other team members effectively. In their article, A Regulatory Model for Transitioning Newly Licensed Nurses to Practice, Nancy Spector and Marcy Echternacht discussed the important aspects of nurses needing to practice and be ready when transitioning to work floors. One of the elements was communication and teamwork. The mortality rate is lower in hospitals where collaborative communication took place compared to hospitals where collaborative communication did not occur (Spector & Echternacht, 2010). Therefore, my communication weakness plan is to write two SBARs about two different patients during each clinical shift and practice it with my preceptor. I plan to achieve my goal by selecting two patients, reviewing their charts, creating SBAR forms, and calling on the preceptor for discussion. 3 To achieve my goals, I will need resources such as patients’ medical records. In the medical record, I will most likely find the patients’ medical history, past and current diseases, surgical procedures, social history, lab values, and medications, and other related information. I will also need to meet with the patients and do a physical assessment to have baseline information. I believe my two goals are related to each other. Nurses rely on EMR during their work. It can reduce medication administration errors, enhance the legibility of physicians’ orders, reduce documentation time, and improve clinical judgment. In their research, Miller and colleagues found that some factors influence EHR skills. Some of those reported by the new nurses and nurse managers were age, previous clinical experience, hospital orientation, and department-specific orientation (Miller et al., 2014). During my past clinical experiences, I had used the Epic system to chart and review my patients’ charts. However, we did not have any prior experience with that system; neither went on an orientation. I had to spend some of my hours figuring out where I could possibly find the information I needed. I faced challenges navigating myself through that system and yet did not know many things about it. In the future, if I would get hired in a hospital that uses a system that I am not familiar with and without providing any training, my patient care would be less officiant, and I would possibly create medical errors. Although we have not started going to Aurora Las Encinas clinical site yet, we have gone to the oriantaiton, which I found beneficial. As a new grad strategy to use EMR, I would highly attend training if available, and if not, I will try to research to get some knowledge about their system. I will also communicate with the informatic technologist personnel for any clarification.
