Homelessness is a significant public health issue in the United States. According to the U.S Department of Housing and Urban Development, California estimated over 151,000 experiencing homelessness, and San Diego had the fourth most homeless population in the United States. Homeless individuals are relatively at higher risk for mortality, morbidity, and chronic illnesses due to lack of housing, lack of access to primary care, and poor hygiene, among other factors (Kushel et al., 2002). Therefore, the homeless population is hospitalized at a far higher rate than the national average and are frequent utilizers of the emergency department services, resulting in increased medical costs (Kushel et al., 2002; Feldman et al., 2017). Healing Hands is a program that will be based in San Diego, California, and will tend to the homeless population. The homeless population is at higher risk of acute and chronic wound infections due to poor health, poor nutrition, uncontrolled diabetes, medication non-compliance, and poor hygiene, consequently exacerbating chronic illnesses and wound infections (Dauby, 2019). Therefore, the program’s focus will be to assess patients’ healthcare needs, signs and symptoms of infections, educate about the importance of hygiene and provide them with hygienic products, provide basic care such as dressing change, and finally refer patients to follow up with their primary care or send them to the emergency department if needed. The program’s mission will be to tend to the homeless population’s health needs by treating their wounds, ulcers such as diabetic foot ulcers, pressure ulcers, skin tears, and preventing worsening of the infections that lead to frequent hospitalization. Healing Hands will be a “mobile clinic” type of a program based in San Diego East County and will work cooperatively with Sharp Grossmont Hospital and nursing schools around the San Diego area. Sharp Grossmont Hospital Emergency department is the only emergency department in East San Diego County and one of the busiest due to its geographic location and being the only hospital serving El Cajon, Santee, and Lakeside areas. As the homeless population continues to rise in San Diego East County, the emergency department at Sharp Grossmont hospital is being overwhelmed with homeless people facing various health issues. One of the most common and recurring hospital visits is contributed to sepsis, a systemic infection that could be caused by a lack of proper treatment of minor and major wounds. The Quality and Patient Safety Report Sharp Grossmont Hospital released in December of 2019 demonstrated that 15.3% of readmission rates for 2019 were patients with sepsis diagnosis. The recurrent admissions strain the emergency room, often leaving injured patients waiting on gurneys stocked up in the hallways. Healing Hands will also work with nursing schools around the San Diego area and provide volunteer opportunities for all nursing students. Grossmont College school of nursing and San Diego State University are within a few miles from Sharp Grossmont Hospital. Both schools require their nursing students to complete specific volunteer hours and public health service hours. Healing Hands will utilize professional medical volunteers, PAs, NPs, RNs, medical students, and nursing students under licensed personnel’s supervision.
