Avoiding tobacco products, get immunizations, and take the necessary medication (Johnson, 2018). When the nurse, patient, and family collaborate on the plan care, the needs, goals, and strategies towards patient well-being, the cure is more achievable because both patient and family have acquired the knowledge for a better outcome patient’s health. When the patient is aware of the expected outcome, the patient is willing to work towards achieving that goal for a better tomorrow. On the other hand, when a patient is not aware of the outcome, he or she will not have the motivation to achieve any goal. When a patient is not aware that he or she needs to exercise due to a high cholesterol level, teaching will not occur. The patient should be taught the reason to modify behaviour before learning can take place. All these measures will help reduce the chances of readmission.
