In completing this project, you will: Review of content related to the transitional care model and research. Interview healthcare professional responsible for the transition of elderly clients from one health care setting to another. Write an essay of approximately 800 – 1000 words summarizing your analysis of the transitional care process. Preparation In preparation for completing this project, complete the following tasks: Read the assigned textbook chapters, online resources, journal articles, and watch the assigned videos as listed in the Module 6 Intro, Readings & Activities page. As you read/watch these resources, ask yourself: • What does the research say works? • What are the problems that can occur in the transitional care process? How do you determine who to interview? First consider what type of patients this person focuses on.
