Enables the patient to live a pain free life for as long as possible and to manage other symptoms so that the person’s last days can be spent in their place of residence with quality and dignity, surrounded with family and friends. Description of current role: hospice RN case manager. My current definition: Health promotion is a practice that seeks to holistically improve the well-being of all people by bettering health education, communities, access to care, and quality of care. The goal of health promotion is to ensure that every individual receives care, optimizing their quality of life without prejudice or discrimination. Agents involved in health promotion programs aim to empower communities and individuals so that they may implement healthy behaviors that reduce the risks of developing diseases. For health promotion to be accomplished, it is imperative that diseases and illnesses be prevented through risk reduction.
I would like to be a part of policy change within our organization or an organization to specifically make changes to increase employee retention and satisfaction.
-It is expensive and is an investment to onboard new employees.
-It is important to make new employees feel welcomed and to give them the necessary tools for success in their jobs.
-It is important to give clear direction and hold employees accountable.
-Current employer has high turnover due to constant leadership change.
-Compensation policies that are fair need to be implemented such as a competitive on call pay and mileage reimbursement. We are currently at 32 cents a mile and the IRS standard is around 56 cents. Many people leave due to higher pay.
-In order to promote health a reduce risk within our organization in order to protect our patients, we need consistent and effective leadership that will adhere to state and federal guidelines. All new staff needs to be educated and that education needs to be repeated yearly, as we do not currently have anyone in a formal position to be responsible for this. Like me, some of the other case managers are asked to have new employees ride with them to learn the job but this is not always fair to the RN case manager, the new employee or the patient as it is unorganized and can be uncomfortable for the patient and their families at times.
Health promotion theory- The socio-ecological theory can assist when thinking about changing policy to create an effective new employee Hospice RN education onboarding plan by not only understanding the new employee’s perception and ideas of health, but to help promote their health and communities so that they can be balanced and well to transfer that understanding to their patients. Health promotion assessment, evaluation and plan- A part of the hospice patient’s receipt of adequate care is determined on that of the assessment, evaluation and plan based on their current individual and familial needs. Each member of the collaborative team must work together to ensure that all areas of need are addressed and a plan is made and implemented. A collaborative team meeting held by team leaders and the educator will meet to review the outcomes based on patient outcomes, family satisfaction and staff retention to determine effectiveness of educational program of employees.
Communicating health promotion interventions: In order to communicate health promotion interventions, leaders will hold meetings to collaborate with staff weekly once a week to assess staff satisfaction as well as hospitalization rates, infection rates, wounds, falls, family and patient complaints. We will also come together to celebrate lives touched, positive stories and recognize staff.
Strengths and limitations: Strengths: Policy and procedural change will improve as evidence by increased and effective communication, increased staff retention rates and satisfaction, decreased patient hospitalizations and infections, wounds and falls. Family, state and federal surveys will be scored high. Patient census will increase despite patient discharges due to deaths as expected.
Limitations: Barriers to proposal include adequate funding from board of directors and stakeholders to
hire a full time educator. Currently, field staff and clinical managers are educating staff which is proving to be stressful for all and is causing not only staff burn out, dissatisfaction but increased risks and negative outcomes for hospice patients. Additional barriers include funding for educational supplies and outdated pay scale. I would like to create a proposal based on my experience on the front lines to present to the board with statistics and an estimated return on investment with creation of the corporate educator role and restructuring of the pay scale, as I believe that once the statistics are created and presented the board will appreciate the investment and see the value of my plan as a leader.
